Question 11 mark

A 68-year-old with advanced cancer has persistent severe pain despite paracetamol. They are opioid-naive, can swallow tablets and have normal renal function. What is the most appropriate plan?

Immediate-release oral morphine can be titrated against pain and adverse effects, with review and a laxative plan from the outset. A long-acting preparation alone is harder to titrate when initiating treatment; waiting until pain is unbearable and omitting constipation prevention are poor plans. A fentanyl patch is not a routine first opioid for an opioid-naive patient.

A single-best-answer question tests starting an oral opioid and planning for constipation.
